Start with how an item connects. Check that the plug on a cable or forceps matches the ESU you use, because connector styles differ between electrosurgery equipment systems. For bipolar forceps, compare the overall length, the tip size and whether the tips are straight or angled. For laparoscopic electrodes and forceps, look at the working length and outer diameter. Kleppinger forceps and inserts vary in jaw pattern, so compare the jaws in the photos and any model number in the title.

What is an electrosurgical unit?

An electrosurgical unit, often called an ESU or electrosurgical generator, is the console that supplies high-frequency current to electrosurgical instruments. The current alternates at radio frequencies, typically between 100 kHz and 5 MHz. Passing that current through tissue generates heat. Generators offer a cut mode and a coagulation mode, and some add blended cut settings. The front panel may carry separate receptacles for monopolar accessories, bipolar accessories and a patient return electrode. Activation comes from a switch on the handpiece or from a footswitch. When you compare units, note which outputs each one has and which accessory plugs its receptacles accept.

What are bipolar forceps?

Bipolar forceps are electrosurgical forceps whose two tips act as the active electrode and the return electrode. Current flows only between the two tips. For that reason bipolar work needs no separate grounding pad. The forceps connect to the bipolar output of a generator through a bipolar cord. A listing may offer the forceps alone or with a cord, so check the title and photos. Plug styles also differ by activation type. One Valleylab generator manual describes 3-pin plugs for hand-switching bipolar accessories and 2-pin plugs for footswitch-activated ones. Compare the pins on the forceps and any cord with the receptacle on your generator.

What does an electrosurgical footswitch do?

An electrosurgical footswitch activates the generator output by foot, as an alternative to a switch on the handpiece. Valleylab's Force 2 manual gives one example. Its monopolar footswitch has two treadles, and the pedals are labeled for cut and coag. The bipolar footswitch has a single treadle. Each one plugs into its own receptacle on the rear panel. That manual gives the monopolar footswitch a 4-pin connector and the bipolar footswitch a 3-pin connector. Instruments with no hand switch rely on a footswitch for activation. Match the footswitch model and connector to the generator you use before you buy one.
